The Benefits of Active Middle Ear Implant Using Stapes Coupler in Mixed Hearing Loss

¸ñÀû: All the types of hearing loss including sensorineural, conductive, and mixed hearing loss are known to be possible indications for Vibrant Soundbridge (VSB). FMT application in stapes was reported in hearing loss with missed incus due to chronic infection or congenital anomaly. Ossiculoplasty using partial ossicular replacement prosthesis (PORP) has same indication with VSB using stapes coupler. Here, we analyzed the audiologic outcomes of stapes vibroplasty and compare the outcome with PORP ossiculoplasty. ¹æ¹ý:From Dec, 2011 to June 2015, 10 patients were implanted an FMT-coupled middle ear prosthesis on the suprastructure of stapes and 18 patients were implanted PORP prosthesis. All patients underwent pure tone audiogram before and 3months after surgery. The hearing gain in each group was analyzed with paired t-test and compared between groups with ANCOVA. °á°ú:In VSB groups, implantees showed significant functional gains through all frequencies. The air conduction pure tone threshold was averagely improved from to 72.3 ¡¾17.6dB to 38.8¡¾5.8dB (p<0.01). In PORP group, the air conduction pure tone threshold was also significantly improved from to 67.7¡¾14.8dB to 54.5¡¾15.9dB (p<0.05). Both group showed hearing gain but the degree of improvement was much higher in VSB group (33.5¡¾16.8dB) than PORP group (13.2¡¾17.1dB) (P<0.01) and VSB group even showed Air-Bone conduction reversal. The WRS were not so different in both group (P>0.05). °á·Ð:In mixed hearing loss, stapes vibroplasty is more effective than PORP ossiculoplasty in all frequencies and even showed air-bone threshold reversal. Audiologic benefit is specifically significant in mid to high frequencies.
