Post-traumatic vertigo associated with intralabyrinthine haemorrhage

Dizziness and vertigo are common symptoms following head trauma. Although these symptoms are resolve within a few weeks in many patients, in some patients it may last much longer. Causes of persistent vertigo after head trauma may include benign paroxysmal positional vertigo, vestibular nerve injury or damage to the utricle or the saccule. Here, we report a 67-year-old woman suffered from post-traumatic vertigo lasting for 4 months, whose MRI scans were compatible with intralabyrinthine haemorrhage. She presented position-induced vertigo, which corresponded to benign paroxysmal positional vertigo involving multiple semicircular canals. The patient has taken canalith repositioning therapy every week, targeting the involved semicircular canals inducing the strongest positional nystagmus and vertigo. Four months later, her vertiginous symptom disappeared, and so did the intralabyrinthine haemorrhage.
