To report on the result of the Vibrant soundbridge (VSB) surgery in patients with unilateral congenital auricular atresia(CAA)who had previously canaloplasty operation.15 years old female patient was presented with CAA. Preoperative average pure-tone threshold was 61.3dB for air conduction and for 10dB bone conduction on the right. Initial CT scans revealed dysplastic ossicle with bony fusion to the adjacent tympatic wall and stapes was intact. Even though she underwent canaloplasty surgery, there was no hearing gain. For hearing gain, she underwent middle ear implantation surgery and subtotal petrosectomy. Implant surgery with the floating mass tranceducer was coupled to the stapes head and there was no complication during surgery. Pure-tone air conduction thresholds were improved by 45dB (preoperation, 61.3dB; postactivation, 16.3dB) at 1 year follow up.In patients with CAA and ossicular anomaly, VSB implantation performed by placing the transducer coupled to the stapes head demonstrated significant improvement in hearing and verbal communication ability.The VSB provides a new rehabilitation option for atresia patients.
