A case of poorly differentiated adenocarcinoma with squamoid feature of submandibular gland

Adenocarcinoma, not otherwise specified (adenocarcinoma, NOS or ANOS) is a malignant neoplasm of the salivary glands with ductal differentiation but otherwise lacks the histologic features that define other type of salivary carcinoma. Tumors show variable histologic aspect and the prognosis is worse than other salivary gland carcinoma. A 51-year-old male presented with a 2*2cm-sized submandibular mass. Fine needle aspiration was done at the lesion and malignant cells were confirmed. Submandibular gland resection and Left modified radical neck dissection type III was done. The final histologic diagnosis was Malignant tumor, poorly differentiated, favor adenocarcinoma with squamoid feature. Mitotic count was 28/10HPF and perineural invasion was founded with focal necrosis. Lymph nodes metastases was also reported ; 31 out of 54 lymph nodes. Squamous portion was confirmed through special stain with p63 and CK 5/6. This atypical adenocarcinoma is thought to be as ANOS. Multiple lymph node metastases of the case also shows invasive characteristics of high grade ANOS.
